THE MY SPA MY WAY MEMBERSHIP CERTIFICATE PURCHASED HEREIN IS VALID FOR ONE YEAR FROM THE DATE OF PURCHASE, UNLESS OTHERWISE SPECIFIED. THE VALUE OF THE MEMBERSHIP CERTIFICATE IS NON-REFUNDABLE AND NON-TRANSFERABLE. THE CERTIFICATE MUST BE PRESENTED UPON ARRIVAL.
THE FULL NAMES OF THE PURCHASER AND THE RECEPIENT MUST BE FULLY DISCLOSED TO THE GUEST SERVICES REPRESENTATIVE AT THE TIME OF PURCHASE AND NOTED ON THE MEMBERSHIP CERTIFICATE IN ORDER FOR IT TO BE REDEEMED.
UPON ACCEPTANCE AND PAYMENT FOR THIS MEMBERSHIP CERTIFICATE, THE CERTIFICATE CAN ONLY BE REDEEMED FOR THE EXACT DOLLAR AMOUNT OR THE SERVICES LISTED. THE MEMBERSHIP CERTIFICATE AND ITS REDEMPTION VALUE OR SERVICES ARE NON-REFUNDABLE AND NON-TRANSFERABLE, UNLESS OTHERWISE DETERMINED BY THE COMPANY OWNERSHIP, MANAGEMENT OR VESTED BOARD OF DIRECTORS.
MY SPA MY WAY REQUIRES 24-HOUR NOTICE TO CANCEL ANY RESERVATION. IF THERE IS A NEED FOR CANCELLATION AND PROPER NOTICE IS NOT GIVEN, THE TOTAL VALUE OF THE GIFT CERTIFICATE WILL BE FORFEITED TO COVER CANCELLATION FEES TO ENSURE THAT SERVICE TECHNICIANS ASSIGNED TO PERFORM THE SCHEDULED SERVICES ARE COMPENSATED FOR THE LOSS OF A SERVICE THAT MAY HAVE OTHERWISE BEEN SCHEDULED.
IF A MEMBERSHIP CERTIFICATE IS ISSUED AS PART OF A SPECIAL PROMOTION, THE CERTIFICATE WILL ONLY BE VALID FOR THE DURATION OF TIME DETERMINED BY COMPANY MANAGEMENT AND OWNERSHIP. TYPICALLY THIS TIME PERIOD IS FOR A MINIMUM OF 30 DAYS AND A MAXIMUM OF 90 DAYS. IF THE MEMBERSHIP CERTIFICATE IS PART OF A MY SPA MY WAY PROMOTIONAL OFFER, IT WILL BE NOTED ON THE "MEMBERSHIP PACKAGE" LINE OR ON THE REVERSE SIDE OF THE CERTIFICATE.
REGARDLESS OF THE TYPE, NO MEMBERSHIP CERTIFICATE CAN BE USED IN COMBINATION WITH ANY ADDITIONAL DISCOUNTS OR OTHER PROMOTION OFFERED BY MY SPA MY WAY AT THE TIME OF REDEMPTION.
IT IS RECOMMENDED THAT THE RECIPIENT OF THE MEMBERSHIP CERTIFICATE MAKE A RESERVATION A MINIMUM OF ONE WEEK BEFORE THE DESIRED DATE THAT THEY WISH TO RECEIVE SERVICES. MY SPA MY WAY DOES ACCEPT WALK-IN GUESTS USING MEMBERSHIP CERTIFICATES, BUT THE ABILITY TO SERVICE THESE CLIENTS IS BASED ON TECHNICIAN AVAILABILITY AND THE AVAILABILITY OF TREATMENT SPACE.
